Get D9325 from dva.gov.auForm D9325 appoints a third party — an advocate, ESO, legal firm, or other representative — to act on your behalf with DVA. Both you (the veteran) and your representative must complete and sign this form.
This is the formal authority that allows your representative to contact DVA, access your information, and in some cases lodge claims or receive payments on your behalf.
Organisations submit the signed form through MyOrg. Individual representatives can submit through MyService. Veterans can also appoint a representative directly through their own MyService account.

- Both parties must sign — the form is not valid with only one signature
- You can grant different authority levels to different representatives
- Multiple representatives can hold "Enquire" authority at the same time
- You can revoke authority at any time by contacting DVA or through MyService
- If your representative is part of an organisation, their ABN must be included
- This form covers family members and support persons too — no separate form is needed for helpers
Common Mistakes to Avoid
- Only one party signing — both veteran and representative must sign
- Selecting the wrong authority level — "Enquire" only lets the representative ask questions, not act
- Not including the organisation ABN when the representative acts through an ESO or legal firm
- Confusing D9325 (who can represent you) with D2139 (what information DVA can collect and share)
