Get D9287 from dva.gov.auForm D9287 is a structured way for your GP or specialist to provide DVA with a confirmed diagnosis. Your doctor fills this form in — not you.
This form is one option for providing diagnosis evidence. Your GP can also provide the same information in a letter on their practice letterhead — DVA accepts both.
From 31 March 2025, a confirmed medical diagnosis is required for a complete claim. Without it, DVA may place your claim on hold at intake.

- Your GP can use their practice letterhead instead if they prefer — the same information is needed either way

- If your doctor is unsure about the form, they can call DVA Health Provider Line: 1300 550 457
Common Mistakes to Avoid
- Trying to fill in the medical sections yourself — this is for your doctor
- Accepting a "working diagnosis" or "suspected" diagnosis — DVA needs a confirmed diagnosis
- Not bringing the form to the appointment
- Confusing date of onset (when symptoms started) with the date of the event that caused the condition
