Form D9287 is a structured way for your GP or specialist to provide DVA with a confirmed diagnosis. Your doctor fills this form in — not you.

This form is one option for providing diagnosis evidence. Your GP can also provide the same information in a letter on their practice letterhead — DVA accepts both.

From 31 March 2025, a confirmed medical diagnosis is required for a complete claim. Without it, DVA may place your claim on hold at intake.
