Get D9213 from dva.gov.au

Form D9213 is used to apply for Non-Liability Health Care (NLHC) for mental health conditions. Any current or former ADF member with at least one day of full-time service can access treatment for any mental health condition — no diagnosis is required.

D9213 can be submitted through MyService, by phone, by email, or in person. MyService is often the fastest pathway to getting DVA-funded mental health treatment.

Note: Cancer NLHC uses a separate form (D9215) with different eligibility rules and lodgement methods.
