Form D2049 is a two-page attachment that you submit alongside your MRCA claim form (D2051) or DRCA claim form (D2020). It is not a claim form on its own — do not submit it without the main claim form.

Page 1 is completed by you (the veteran). Page 2 is completed by your treating doctor or GP.

This form helps DVA understand the specific injury or disease event and its clinical history. It is distinct from the Diagnosis Form (D9287), which focuses only on confirming the diagnosis. D2049 covers the broader narrative — what happened, when, and the clinical timeline.

  • Complete page 1 before your GP appointment — your doctor only needs to fill in page 2
  • This form goes with your main claim form (D2051 for MRCA or D2020 for DRCA) — do not submit it on its own
  • Be specific about dates and locations in your veteran section — vague descriptions are harder for DVA to assess
  • This is different from D9287 (Diagnosis Form) — D9287 confirms the diagnosis; D2049 describes the injury or disease event and clinical history
  • If you are claiming multiple conditions, you may need a separate D2049 for each

Common Mistakes to Avoid

  • Submitting D2049 without the main claim form (D2051 or D2020) — it is an attachment, not a standalone form
  • Leaving page 2 blank — your doctor needs to complete the clinical section
  • Confusing D2049 with D9287 — D9287 is for diagnosis confirmation only; D2049 covers the broader injury or disease narrative
  • Being too vague about the service connection — include specific events, dates, and locations
  • Not realising this form exists — many veterans submit only the main claim form without this useful attachment
