Get D1360 from dva.gov.auForm D1360 is for claiming incapacity payments — financial support when your DVA-accepted condition prevents you from working at full capacity.
Payments are based on the difference between your Normal Weekly Earnings and what you can currently earn.
From 31 March 2025, DVA checks claims for completeness at intake. A complete claim requires this form fully completed, correct proof of identity, and a medical diagnosis. If any of these are missing, your claim will be placed on hold — but your lodgement date is still registered. Use our Evidence Builder to check you have the mandatory items ready.
